Probody Wellness Clinic (PWC) Hormone Replacement Therapy (HRT) Membership Terms of Service Agreement

Effective Date: The date of your first subscription payment to Probody Wellness Clinic (PWC).
Clinic: Probody Wellness Clinic (“PWC,” “we,” “our,” or “us”)
Client: (“You,” “Member,” or “Client”)

1. Membership Options

By enrolling in the PWC Hormone Replacement Therapy (HRT) Program, you agree to participate in one of the following membership plans:

  • Standard HRT Plan — $139/month: Includes injectable hormone therapy, supplies, and provider management.

  • Oral HRT Plan — $169/month: Includes oral hormone therapy with provider oversight and ongoing support.

Membership plans require a 12-month minimum commitment, billed automatically each month.

2. Bloodwork & Follow-Up

Follow-up bloodwork is required three (3) months after initially joining the program to assess prescribed treatment and then every six (6) months. Follow-up bloodwork is billed separately at $100. Members acknowledge that maintaining updated lab work is a condition of continued participation in the HRT program.

3. Payment Authorization

You authorize PWC to charge your payment method for monthly HRT membership fees. Payments will be billed automatically each month for the 12-month commitment period.

  • You may update your payment method by contacting PWC directly.

  • Payments are non-refundable once processed.

4. Medical Disclosure

You confirm that all medical information provided to PWC is accurate and complete. You agree to promptly report any side effects, health changes, or adverse reactions during your care and to follow medical recommendations as provided by your healthcare provider.

5. Cancellation & Early Termination

  • Membership includes a 12-month minimum commitment, billed monthly.

  • An early termination fee of 50% of remaining commitment will apply if therapy is discontinued before the 12-month term (i.e. 9 months remaining equals a $625.50 termination fee), unless discontinuation is due to a verified adverse medical reaction.

  • Payments already processed are non-refundable.

6. HIPAA Privacy Acknowledgment

PWC complies with all applicable HIPAA (Health Insurance Portability and Accountability Act) regulations. Your health information will be securely used and stored in compliance with federal privacy standards. You may request a copy of PWC’s HIPAA Privacy Policy at any time.

7. Telemedicine Consent

You consent to receive medical consultations, follow-ups, and treatment adjustments via secure telemedicine platforms when deemed appropriate by your provider.
You may withdraw consent for telemedicine services at any time by submitting a written request.

8. Electronic Signature & Agreement

Payment of this invoice constitutes agreement to the Probody Wellness Clinic HRT Membership Terms of Service - you acknowledge and agree to:

  • The terms and conditions of this PWC HRT Membership Agreement,

  • The automatic monthly billing for the selected membership plan,

  • The consent to telemedicine and HIPAA-compliant communication.
